In accordance with law, the district must provide parents with written notice of each school-based health-related service offered at the campus their child attends. These routine services promote student safety, wellness, and readiness to learn. The services may be provided by qualified school staff, including nurses, counselors, our China Spring social services team, and athletic trainer.

This consent does not take the place of an individualized health plan, 504 plan, or other legally required document. A parent has a right to withhold consent for or decline any health-related service. Listed below are examples of some of the health-related services that are included in this consent. If you do not consent your student will not have access to these services per Senate Bill 12.

First aid and injury evaluation
Support of students feeling sick
Administration of over-the-counter and prescription medication in accordance with law
Monitoring of chronic health conditions (e.g., asthma, diabetes)
Heat illness prevention and injury support for student athletes

Examples of routine counseling services that are included in this consent. If you do not consent your student will not have access to these services per Senate Bill 12:

Advise students and their parents regarding the importance of postsecondary education, coursework designed to prepare students for postsecondary education, and financial aid availability and requirements
Early mental health prevention and intervention
A guidance curriculum to help students develop their full educational potential, including the students interests and career objectives

Any additional health-related services that the campus may believe are necessary and appropriate will necessitate separate written consent and parental involvement, unless during an emergency or as allowed by law.
